713-442-0427The Centralized Pharmacy Anticoagulation Service (CPAS) is also referred to as “Coumadin Clinic” or “Warfarin Clinic” and is comprised of anticoagulation specialized clinical pharmacists and staff who work closely with your doctors to help manage your warfarin therapy. They assist with lab monitoring, warfarin education, drug interactions, warfarin dose adjustments, and warfarin refills. Patients who are on warfarin must have routine labs to ensure their warfarin dose is safe and effective.
Centralized Pharmacy Anticoagulation Service (CPAS)

Kelsey-Seybold Clinic provides anticoagulation services to monitor and assist patients who take a medication called warfarin (also known as Coumadin® or Jantoven). Warfarin is an anticoagulant, or “blood thinner,” that is used to help treat or prevent conditions such as, but not limited to:
- Atrial Fibrillation (abnormal heart rhythm)
- Clotting disorders
- Clot in the leg (deep vein thrombosis or DVT)
- Clot in the lungs (pulmonary embolism or PE)
- Mechanical heart valves
- Previous stroke or heart attack
Enrollment
- A CPAS team member will call to schedule a phone appointment with a clinical pharmacist.
- They will review your medical history as it relates to warfarin, review your medications to ensure your chart is updated, provide warfarin education, and give instructions on your warfarin dose & next International Normalized Ratio (INR) lab date.
INR Monitoring & Dose Adjustments
- Once you complete your INR labs, you can expect a call from CPAS to review your INR results within 24 to 72 hours and provide adjustments to your warfarin if needed.
Reminders
- You will receive a call or text reminder 2 business days before you are due for your INR labs. You will also receive additional calls if you are overdue for your INR.
