Advanced Wellness Exam

The program may be direct billed via a corporate sponsored program, or coordinated with your health plan. Kelsey-Seybold Clinic accepts more than 50 health plans. There is a $250 extended wellness fee (not covered by insurance). Any applicable co-pays or co-insurance amounts due may be direct billed to the company or paid by the individual.

The Advanced Wellness Exam includes highly personalized concierge-level service, plus:

• A designated representative who personally meets and greets you when you arrive.
• Personalized assistance with scheduling appointments for referrals to other physicians and for follow-up tests, recommended screenings, visits and repeat physical exams as directed by the physician.
• Enrollment assistance in MyKelseyOnline to ensure that you can email questions to your physician and view most lab and test results.
• Personalized assistance with insurance eligibility or billing issues.

• A comprehensive physical examination, including a 45-minute visit with a physician to discuss health concerns and answer questions.
• Comprehensive lab testing, with same-day test results for most lab work.
• EKG or stress test (based on age).
• A 30-minute consultation with a registered dietitian.
• Audio and visual screening.
• The Know Your Number® Health Risk Assessment, a sophisticated risk analysis indicating your personal five-year risk for development of diseases, including stroke, diabetes and heart disease.
• Catered light breakfast in a private dining room.

Our patients have the option of selecting the physician completing the Advanced Wellness Exam as their personal physician. The concierge will assist with all follow-up appointment scheduling or any visits for illness as needed.
