Diabetes Management. Diabetes is a complex group of diseases marked by high blood glucose (blood sugar). Left unmanaged, diabetes can lead to serious complications, including heart disease, stroke, hypertension, blindness, kidney disease, diseases of the nervous system, amputations, and premature death. Management of Hemoglobin A-1c is key to successfully managing diabetes.

Hypertension Management. Controlling high blood pressure is an important step in preventing heart attacks, stroke, and kidney disease, and in reducing the risk of developing other serious conditions. High blood pressure can be controlled through prescribed medications, low-sodium diets, increased physical activity, and smoking cessation.

All HEDIS comparison data is compiled from the National Committee for Quality Assurance (NCQA) annual State of Healthcare Quality Report.

NCQA produces The State of Health Care Quality Report every year to focus on key quality issues the United States faces and to drive improvement in the delivery of evidence-based medicine. This report documents performance trends over time, tracks variation in care and recommends quality improvements.
